All students traveling on any trip with our ministry must complete a Parent Permission, Medical Consent and Medical Information Form and return to one of our Student Ministry directors prior to a departure. No student will be allowed to travel on a Student Ministry sponsored trip if a completed form is not on file. Please note that this form MUST be notarized. Covenant has a notary on whom is available to notorize during the week: Monday-Thursday from 8 am-5 pm and on Fridays from 8 am-1pm.
The Student Ministry respects the privacy of the information provided on this form. As such, please rest assured that this information will not be shared with anyone and will only be accessed in case of any emergency with a student. This form only needs to be completed once for each student and will be kept on file for future trips, unless the information provided changes at which time a new form must be completed.
